How do we use your information?

How we collect your information

We may collect your contact and basic information when you submit either a contact form or a quote request form online. We place these forms on Facebook advertised through our Business Page 'Surpass Financial Services' or from the forms found on this website. Although we can be found on other platforms like Twitter and Google, we do not have active contact or quote forms anywhere other than Facebook and this website. These are the only two avenues we would as you to submit forms. The other ways we may request information from you is in correspondence over the phone or email. Whenever you are speaking to an agent or other personnel, always ask for their license number and write it down for your records and security. Now, what do we do with your information?

HOW DO WE USE YOUR CONTACT INFO?

Contact information is submitted with every form so that we're able to contact you in regards to what information you're requesting. This may include your phone number, email, full name, city, or state. We only ask you for your full address if we are completing an application, in which case it is required. 

We are not a vendor service nor do we supply other companies or persons with inquiries or leads. Therefor, we never sell your contact information to any third party. We only use your contact information to contact you in regards to your inquiry and present or future products or services that may be available to you.

HOW DO WE USE YOUR PERSONAL INFO?

Personal information may be submitted with a form so that we're able to provide you with relevant information and/or quotes as soon as possible. This may include but is not limited to general information about your hobbies, smoking habits, financial needs, and/or family size and needs. 

We are not a vendor service nor do we supply other companies or persons with inquiries or leads. Therefor, we never sell your personal information to any third party. We only use your personal information to assist us in contacting you with relevant information, quotes, and present and future products or services that may be available to you. 

HOW DO WE USE YOUR MEDICAL INFO?

General medical information may be required on some submission forms in regards to things like your smoking habits, if you're currently on medication, if you've recently been hospitalized, or if you are currently disabled. We do not ask specific medical information on any submission forms. We only ask specific medical information when completing a formal application together. This mean if we are completing an application together, we will need to gather very specific medical information in order to process the applications. This may include but is not limited to medication and dosage, recent hospitalizations and causes, and diseases and disorders.

We are bound by HIPAA law which means we legally cannot share your medical information with any third party. When an application is submitted, our underwriters have the right to review and request information from your medical history and/or physicians in order to obtain all information needed to approve or deny a policy. This is the only reason we may request or review information. This information cannot be shared, sold, or used to market or advertise. There is a HIPAA page included in all application which outlines this law and how we use your medical information. 

WHO ARE WE TO ASK YOU FOR THIS INFO?

We are a team of licensed Life, Accident, & Health Insurance Agents contracted as providers for life insurance companies and their products. 

Each agent has a National Producer Number (NPN) as well as a License Number for the specific state that they are doing business in. We provide you with both of these numbers at the beginning of each consultation or meeting. You can use the National Association of Insurance Commissioners Lookup System to find your agent using these numbers. 

Each agent also has a Company Agent Number that is identifiable by the carrier of the products. This agent number is always electronically stamped onto all applications you submit and will also be provided to you by your agent. Aside from being bound by the rules and regulations of the insurance commission, agents are also bound to the rules and regulations of the individual life insurance companies. Each agent's license relies on their adherence to these rules, laws, and regulations.
